Ciclosporin Eliminates Refractory Proteinuria Effectively
If our immune system is overactive, it will attack foreign bacteria and kidney cells. Then we will get kidney disease. Protein will leak into urine.
If your condition is not serious, we can take Pril or Sartan medicines for relieving inflammation. If your condition is serious, we need effective medicines to attack immune system to reduce the damage of overactive immune system response on kidneys.
The effective medicine is Ciclosporin.
Let’s first talk about its side effects.
Ciclosporin has been used in nephropathy for more than 30 years since its clinical application in 1987.To be honest, we are not very willing to treat kidney disease with cyclosporine because of the high side effects.
Patients all know that ciclosporin is a kind of immunosuppressor. After using it, your immunity will be reduced. Cyclosporine A suppresses the overactive immune system by reducing the production of T lymphocytes by reducing some cytokines in the body. In addition to reduced immunity and increased risk of infection, cyclosporine is also toxic to the kidneys. But kidney patients do not need to worry about it too much. The dosage of cyclosporine below 5mg of per kg of body weight, is safe.
Other side effects include high blood pressure, abnormal metabolism, infection, thrombotic microangiopathy, gum hyperplasia and gastrointestinal reactions, etc.
Although cyclosporine has many side effects, we still need it in severe cases.
Which kidney disease can be treated by cyclosporine?
1. Minimal Change Disease (MCD)
If MCD patients have hormonal contraindications, such as obesity, diabetes, osteoporosis, advanced age (over 70 years old), they cannot use or cannot use large doses of hormones.
In addition, some patients with MCD frequently relapse after the use of hormones, it is necessary to add immunosuppressant. If it still relapses after adding cyclophosphamide, it is necessary to consider cyclosporine.
In addition, some patients with MCD do not want to use hormones. Because hormones can cause centripetal obesity, stretch marks, buffalo back and acne. In this case, cyclosporine can be considered. As long as the dose of cyclosporine is well controlled, the therapeutic effect can be achieved and the side effects of hormones can be avoided.
Hormone therapy is preferred in patients with focal segmental glomerulosclerosis (FSGS). However, some FSGS patients cannot use hormones, or cannot use large doses of hormones, or often relapse after using hormones: two or more recurrence within 6 months, or three or more recurrence within 12 months. In this condition, cyclosporine can be considered.
3. Membranous Nephropathy
If urine protein in Membranous Nephropathy can be negative after conventional treatment., the impact on renal function is not as big as other kidney disease even if the urine protein is 1-3g/24g. However, if the urine protein is greater than 4g, hormone + immunosuppressant therapy should be considered, and hormone alone is not effective in membranous nephropathy.
Cyclosprine has some effects to kidney patients. For your safety, you should ask the doctor before using it. Consult us for more information.
