How to Treat Renal Anemia
In recent years, there are more and more evidences about the epidemiology and treatment of renal anemia. In order to adapt to the development and changes of clinical diagnosis and treatment in China and regulate the management of renal anemia, consensus expert group on diagnosis and treatment of renal anemia from Chinese Medical Association Nephrology Branch updates the consensus on the diagnosis and treatment of renal anemia based on international guidelines.
How to treat renal anemia?
1. Usage and dosage of Iron Prep
Iron supplementation can be divided into initial treatment stage and maintenance treatment stage.
Oral iron-replacement: 200 mg of elemental iron should be given every day, and the status of iron should be evaluated after 1-3 months.
Injection of intravenous iron: 1) A course of iron sucrose or iron dextran is usually 1000 mg (e.g. 100 mg/ time, 3 times per week). After one course of treatment, the iron status has not reached the standard, and another course of treatment can be repeated. 2) maintenance treatment phase: When the iron status is up to the standard, the dose and time interval should be adjusted according to the patient's iron status, reaction to iron agent, hemoglobin level, ESAs dosage, reaction to ESAs and recent complications.
2. ESAs treatment
ND-CKD patients should take ESAs treatment when Hb < 100 g/L.
PD-CKD and HD-CKD patients: since Hb of dialysis patients decreased faster than that of non-dialysis patients, ESAs treatment should be started when Hb < 100 g/L to avoid Hb lower than 90 g/L
Treatment target of HB is ≥115 g / L, but larger than 130g/L is not suggested.
According to the patient's age, dialysis mode and duration of dialysis, the duration of ESAs treatment, and whether the patient is complicated with other diseases, the target value can be adjusted individually.
3. Transfusion therapy
Red blood cell components transfusion should follow the principles below: (1) determine the nature of anemia and the possibility of correction, so that correctable anemia can be treated accordingly; (2) determine that transfusion of red blood cell components can alleviate the corresponding symptoms and signs. If the transfusion of red blood cells cannot reverse the symptoms and signs, transfusion therapy should not be done.
The above treatments are only for symptoms. It can not treat renal anemia from root. Anemia is from injured kidneys.
